ATTENDANT CARE PROGRAM (ACP)

The Attendance Care Program provides financial reimbursement to individuals with long-term or severe physical disabilities who require attendant services such as in-home assistance with personal care, household chores, and transportation. A request for an application can be filled out by using an online form. If you are a representative for the applying person and would like to request a form on their behalf, please fill out the following online form.

I'M BETWEEN 18 - 64 YEARS OF AGE

Below is a list of resources that provide services for individuals with disabilities between the ages of 18 - 64.

  • In-Home Aide Services (IHAS) provides chore, personal care, transportation/escort, training in self-care, and care-giving skills services for persons with disabilities 18 or older.  Guidance and support to parents, caretakers, and to aged and vulnerable adults are also available.  Contact the In-Home Aide Services program at 1-800-332-6347, TTY 1-800-925-4434 or click here for more information on the In-Home Aide Services program.
  • Living at Home Waiver Program is a statewide program for people with physical disabilities, who are between the ages of 18 to 59, and need assistance with daily living.  For more information, call 410-767-7479 or click here for more information on the Living at Home Waiver.
  • Project Home provides a room and board, personal assistance with daily activities, recreational and social activities, medication monitoring, transportation to medical appointments, and supervision.  For more information, contact the Project Home program at 1-800-332-6347, TTY 1-800-925-4434 or click here for more information on the Project Home program.
  • The Waiver for Older Adults provides services for older adults, age 50 or older, to live in their own homes or an assisted living facility.  If you want more information on the Medicaid Waiver for Older Adults, please call the Maryland Department on Aging at 410-767-1118 or locate your Area Agency on Aging at 1-800-AGE-DIAL or click here for more information on the Waiver for Older Adults.
  • The Waiver for Individuals with Developmental Disabilities provides services and supports in the community for individuals with developmental disabilities regardless of age.  Click on the following link to locate the contact information for your Developmental Disabilities Regional Office.
  • The Respite Care Program provides short-term, periodic care to individuals with developmental or functional disabilities, in order to provide a period of rest and renewal to family caregivers.  Contact the Respite Care Program at 1-800-332-6347, TTY 1-800-925-4434 or click here for more information on the Respite Care Program.

I CARE FOR AN INDIVIDUAL 22 YEARS OR YOUNGER

Below is a list of resources that provide services for individuals with disabilities under the age of 22.

  • The Model Waiver for Medically Fragile Children provides services for children with complex medical needs, age birth through age 21, to live in their own homes instead of a hospital, nursing facility, or other long-term care facility.  Contact the Coordinating Center at 410-987-1048, 301-621-7830, TTY 1-800-735-2258 or click here for more information on the Model Waiver Program.
  • The Waiver for Children with Autsim Spectrum Disorder will allow eligible children with Autism Spectrum Disorder to receive additional waiver services to support them in their home and community.  Contact your local school system and ask to speak to the Autism Waiver Coordinator or call the Maryland State Department of Education at 1-800-535-0182, TTY (410)333-6442 or click here for more information on the Waiver for Children with Autism Spectrum Disorder.

I'M 65 YEARS OF AGE OR OLDER

Below is a list of resources that provide services for individuals with disabilities over the age of 65.

  • Senior Care provides case management services and funds for services for people 65 or older for personal care, chore service, medications, medical supplies, adult day care, respite care, home delivered meals, transportation, and emergency response systems.  Contact the Senior Care program at 1-800-243-3425, TTY 1-800-735-2258.
